Name, Address, Phone Number of the Group.E-mail address and web-page (if applicable) of the shelter or rescue.Copy of the Tax Exempt Certificate for the group.
Once PPE has this information on file, we will forward the coupon code (good for 25% off any order) to the registered e-mail address.
